Dr. Omid Fazeli is a USDA-accredited veterinarian. We review your destination requirements, your pet’s vaccination and microchip history, and your travel dates, then help you through the required examinations, testing, documentation, and certification process.
The first step is getting the timing right. We work backward from your departure date to determine what your pet needs and when — including vaccinations, microchipping, blood tests, parasite treatments, health certificates, and other destination-specific requirements.
If your airline has provided a form, bring it with you. Airline requirements can be different from government entry requirements, so we review both.

Call with your travel details
Tell us your destination, travel date, whether you are flying or driving, and your airline if applicable. We check the current requirements for your specific trip.
Examination and documentation
We examine your pet and complete the required veterinary documentation. Some trips can be completed with a single appointment, while others require multiple steps or specific waiting periods.
USDA endorsement, if required
Some destinations require the completed health certificate to be endorsed by USDA APHIS before travel. We will let you know whether endorsement is required and guide you through that step.
